Limitations, Exclusions, and Exceptions

Member is defined as primary cardholder and dependents as defined under the Affordable Care Act.

Providers are subject to change without notice. Programs may vary in some states. Providers and locations may be removed from the network at any time.

This is a discount program only. The program may be cancelled or modified at anytime. You will receive notice if the program is cancelled or materially modified.

The discount company will not reimburse or pay any portion of any provider’s fees. These benefits may not be used with any other discount plan or program. Listed or quoted prices are subject to change without notice.

Providers may offer products or services to the public at prices lower than the discounted prices. In such event, members will be charged the lower price.

Savings are based on the provider’s normal fees. Actual savings will vary by location and the services or products purchased.

This discount program is a referral plan, and makes no warranties concerning the quality of care received. Providers are responsible for the professional advice and treatment provided to members.

New Benefits Privacy Policy

At New Benefits, keeping member information secure is a top priority. By interacting with New Benefits through our websites, mobile applications, products, and services, you consent to the use of information that is collected or submitted as described in this privacy policy. This document outlines our privacy policy and how we protect and use the data you provide to us.

Your Personal Information

The personal information members provide us is collected solely for the purpose of providing members and their dependents (if applicable) with the New Benefits services and benefits. This information is protected by New Benefits by maintaining physical, electronic and procedural safeguards.

Information We Collect

Personal information we collect about members comes from one or more of the following sources: paper application, faxed application, web application, your employer, membership association, insurance carrier, or affinity groups. Personal information collected includes, but is not limited to: name, mailing address, telephone numbers, e-mail address, social security number (when necessary), date of birth, gender, geolocation, mobile device information, dependent name, dependent gender, dependent date of birth, dependent social security number (when necessary), dependent relationship, bank account information, or credit card information.

We may disclose any or all this information based on business need only, to our Affiliates, service providers, third party service providers, provider networks and other suppliers of products and services to New Benefits members.

New Benefits will maintain member information even after membership is terminated for any reason. This information will be protected in the same manner as that of an active member. A member may request in writing at any time after discontinuing the membership to remove the information from our file, subject to business record keeping requirements for administration or reporting purposes.

Sharing Information with New Benefits Affiliates and Service Providers

New Benefits will share member information with its Affiliates, related companies and service providers to the extent it is necessary to provide and manage benefits to the member or their dependents. Unless otherwise permitted by law New Benefits will not share with these parties any information it collects that is not needed by these parties to provide the benefits to the member or their dependents. These organizations are not authorized to use member information for any purposes other than for New Benefits business.

Sharing information with Third Parties or Service Providers not affiliated with New Benefits.

New Benefits will not share, sell or otherwise disclose to any third parties not affiliated with New Benefits member information collected by New Benefits. This includes other service providers and marketing organizations not directly related to providing New Benefits member services.

Request to Know

You have the right to request we disclose certain information to you about our collection and use of your personal information over the past 12 months. Once we receive and confirm the verifiable consumer request, we will disclose to you:

  • The categories of personal information we collected about you.
  • The categories of sources from which the personal information was collected.
  • The categories of personal information that was disclosed for a business purpose about the member.
  • The categories of third parties with whom we share the personal information.
  • Our business purpose for collecting personal information.

Right to Correction and Deletion

You have the right to request we correct or delete any personal information we collected and retained, subject to certain exceptions. Once we receive and confirm the verifiable consumer request, we will correct or delete the personal information from our records and our affiliates records, if necessary.

Right to Opt-Out

New Benefits will not sell your personal information.

Right to Non-Discrimination

You have the right to exercise your privacy rights. You will not be treated in a discriminatory manner for practicing these rights.

Authorized Agent

An authorized agent may make requests on your behalf after submitting proof to us.

Response Time

Upon receiving a request, we will confirm receipt of the request within 10 business days. New Benefits will respond to a verifiable consumer request within 45 days of receipt.

New Benefits Commitment to Members

Keeping our member information secure and using it only as needed for business reasons to provide services to our members is a top priority to New Benefits. We will safeguard, according to strict standards of security and confidentiality, any information provided to us by our members. We will permit only authorized employees who are trained in the proper handling of member information to have access to that information. Employees who violate our Privacy Policies will be subject to our normal disciplinary process.

We will not reveal member information to any external organizations (other than Affiliates and service providers mentioned above), unless the member specifically authorizes us to do so or as required by law. We will always maintain control over the confidentiality of our member information. Whenever we hire other organizations to provide support or services to New Benefits, we will require them to conform to our privacy standards and allow us to audit them for compliance. We will attempt to keep member files complete and up to date and accurate. A member can contact our Member Services Department at any time during business hours to correct or update any information. We will not make available information we have collected in connection with or in anticipation of a claim or legal proceeding.

Changes to Our Privacy Notice

We reserve the right to amend this privacy notice at our discretion at any time.
